• Graduate of an accredited school with a Master’s degree.
• Current licensure to practice as a CNM in the State of Maryland, or the ability to acquire license.
• Unrestricted prescription privileges with current CDS numbers.
• Signed contract with supervising physician in area of practice.
• No history of Medicare or Medicaid fraud.
