JOB SUMMARY:
Reviews medication orders issued by physician or other authorized prescriber to assure accuracy and determine formulas and ingredients needed, assuring that all orders are filled and labeled (including secondary labels) properly. Performs clinical functions as outlined by the Pharmacist in Charge (see clinical outline). Maintains the integrity and accuracy of the drug distribution and delivery systems. Ensures the appropriate and safe utilization of high risk medications and controlled substances as outlined by department and hospital policies. Maintains patient profiles with current information on allergies, diagnosis, weight, current medication and IV therapy orders. Compounds and supervises the compounding of medications and intravenous solutions including hazardous medications using standard formulas and processes. Review patient's drug regimen for potential interactions, interference's or incompatibilities and notify physician or nursing staff as appropriate. Assist in training pharmacy technicians and student pharmacists.

Department/Unit Summary:

About our Department: The Department of Pharmaceutical Services provides comprehensive pharmaceutical care services to Vanderbilt University Hospital (VUH), Monroe Carell Jr. Children's Hospital at Vanderbilt (Children's Hospital), Vanderbilt Psychiatric Hospital (VPH) and The Vanderbilt Medical Group (VMG). This patient-centered service is provided through the Patient Care Center organization and clinic structure. The role of the Pharmacy is to promote safe and effective medication therapy for all patients and to provide oversight and leadership of the medication use system. • Evening shift.
• Shift hours are 9pm-7am M-F and 7pm-7am S/S

Bachelor's Degree (or equivalent experience) and 1 year Hospital experience

Licensure, Certification, and/or Registration (LCR):

• Licensed Pharmacist Required

• Per Pharmacy Department policy, all new hires are subject to pre-employment drug screening (post offer), as well as for cause and random drug screening during their tenure with Vanderbilt.
