These 2 positions are located at the VACT Women's Clinic. These LPN positions will serve as Women's Health Mammography Care Coordinators. There positions will be open to the Public at the GS6 level. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education - Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment OR granted a license by a jurisdiction that does not require graduation from an approved school AND have at least one year of successful practice as an LPN/ LVN. Licensure - Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. GS-06 Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN. Proficiency in initiating, performing, and completing assigned duties in providing care to variable patient populations. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions, using judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognizing, reporting, and recording relevant patient information. Ability to observe, identify, and response to a patient's needs for care including medication, equipment-assisted care, and patient/family education. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for, documents, and reports physical and/or emotional changes in the patient's condition from prescribed medications/treatments. Knowledge and ability to recognize urgent or emergent patient care situations and initiating appropriate emergency interventions as directed. Knowledge and understanding of human behavior, patient motivations and reactions to situations and the ability to appropriately utilize this knowledge in working effectively with patients, family members and other staff. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which includes preparing the patient and assisting in the diagnostic examination, preparing, and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. ["The incumbent functions as the Breast and Cervical Cancer Screening Coordinator, assigned to the VA Connecticut Health Care System and catchment area. Works closely with the Women Veterans Program Manager (WVPM), Women's Health providers, Office of Eligibility and Enrollment, Radiology, Lab, Community Care Office, and outside providers. Incumbent coordinates and facilitates, as needed, women's health care including contacting the Veteran, entering orders by protocol, facilitating appointments, consult tracking and appropriate authorization, tracking receipt of test results and other medical documentation, notifies providers of needed follow up care and assists with care coordination of high-risk diagnosis. The incumbent is a direct report to the Nurse Managers of Primary care for clinical oversight in collaboration with the Women Veterans Program Manager and Women's Health Medical Director. The position is dedicated primarily for Women's Health Mammography Care Coordination. The incumbent provides direct care according to their education, scope of practice and demonstrated competency. Incumbent will work closely with Care in the Community and Primary care providers to assess, and problem solve patient care issues related to the mammography program and educate team and patients as issues are identified. The incumbent is responsible for telephone triage, facilitating appointments and procedures in accordance with patient need and availability related to women's health care. The incumbent should be proficient in computer skills, accurately documenting all care and recommendations, communicating with providers as necessary to assure continuity of care of the women Veteran. The incumbent should be able to work independently in a multifaceted environment utilizing critical thinking skills for complex issues and able to act quickly to ensure all safety measures have been addressed and follow up has been achieved. The incumbent will become competent in specific gynecological and specialty procedures, which may be scheduled at other VA facility as well as the community. Incumbent will be able to telework for a proportion of the duties as appropriate. Collaborates with Primary Care, including the CBOCs and clinics in the community, with a wide variety of duties associated with the women's Health Mammography program. The mammography coordinator will assist the WVPM and the WHMD with quality improvement strategies to meet goal performance measure for mammogram tracking and reporting for all service lines performing primary care for female Veterans. The primary responsibility of the incumbent is to track and coordinate all breast and cervical cancer screening services, including but not limited to mammography, breast MRI, ultrasound and associated biopsies, cervical cytology, and HPV testing done within the VA and by community providers. Supports the Women's Program by participating in Outreach activities including women's health fairs and education campaigns. Supports the Women's Health Program and ensures that women have timely coordinated access to all gender specific services.
