Grade Determinations: GS-09, - Medical Instrument Technician (Cardiac Catheterization) In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments. Experience. At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of cardiac catheterization technician services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Demonstrated Knowledge, Skills, and Abilities. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to the Cardiac Catheterization Laboratory. Knowledge of JCAHO and other regulatory requirements. Ability to communicate effectively and professionally with employees at varying grade levels. Ability to manage and supervise employees. Ability to provide, or provide for, staff development and training. Physical Requirements: See VA Directive and Handbook 5019, Employee Occupational Health Service. ["The incumbent will function as full members of the Cardiac Catheterization Laboratory Team. They scrub, circulate, and operate both electrical and physiologic monitoring equipment as well as single plane / bi-plane fluoroscopic equipment etc., for the most complex examination and treatment procedures including procedures such as endocardial biopsy, cryoablation and radio frequency ablation procedures, intra-aortic balloon pump, and ventricular assist devices, as well as transcatheter aortic valve replacements for very critical patients. They inject contrast manually or with the aid of a mechanical contrast device to visualize anatomical structures during angiography procedures in accordance with physician orders. They mentor and train lower graded technicians, nurses, nursing students, and cardiovascular technology students on cardiac Invasive Cardiology procedures and assist the supervisor/manager with quality control and performance improvement activities. Cardiovascular technicians also write policies and standard operating procedures. Implementation and evaluation of equipment, technology, and new interventional techniques are integral components of the role. Major duties will include, but will not be limited to: 1. Following clinical review by a provider, triages exam requests to give priority to cases that are clinically more urgent and coordinates the completion of inpatient exams with scheduled outpatient exams. 2. Assures all equipment is operational prior to patient procedures; contacts Cath Lab Supervisor / Biomed / Attending Physician when equipment is inoperative or malfunctioning. 3. Secures patient's confidence / cooperation during procedures. Explains the procedure, positions for desired results, cushions and shields patient for a variety of difficult procedures. 4. Properly incorporates appropriate PPE when conducting procedures. 5. Gathers patient information from the electronic medical record. 6. Operates cardiac physiologic monitoring equipment. Sets up and calibrate pressure transducers. Properly identifies, labels and samples intracardiac and vascular waveforms. Completely and accurately documents all aspects of the procedure including capturing and inputting data or procedural calculations in the hemodynamic monitoring system. 7. Preps and drapes clients for invasive / operative procedures. 8. Facilitates insertion and manipulation of guidewires and catheters into vasculature under fluoroscopic guidance for diagnostic and interventional purposes. 9. Obtains invasive hemodynamic data, cardiac output, and blood samples. 10. Injects contrast manually or with the aid of a mechanical contrast device to visualize anatomical structures (angiography, venography, etc.).
