Your day.

As a RN Patient Navigator you need to know how to:

• Utilizes specialized knowledge about specific disease(s) and community resources to triage phone calls and to make appropriate referrals according to protocol.


• Conducts initial telephone intake assessment on all self-referred callers with diagnosed disease(s).


• Facilitates primary coordination, documentation and follow-up related to consultations services.


• Utilizes nursing assessment skills to perform a thorough history, clinical exam and related patient education/instructs patient.


• Identifies clients and family members for nutritional, psychosocial or cancer risk analysis services.


• Identifies patients in psychosocial/medical crises and refers to appropriate physician/health professional in a timely manner.


• Assists with monitoring of the incoming and outgoing requests for x-ray films, pathology slides or other medical records as needed for consultative services, radiology services, and/or patient care conferences as assigned.


• Communicates appropriately with referring physicians to discuss relevant aspects of patient care.


• Collaborates with Surgeons in delivery of direct patient care.


• Assists appropriate medical staff with diagnostic/therapeutic procedures (e.g., Scope).


• Documents all patient interactions appropriately and timely in the medical record.


• Applies legal and ethical principles to nursing practice.


• Reviews medical record and/or patient history to initiate diagnostic tests necessary for care prior to physician visit.


• Reviews and assesses lab and other diagnostic test results to anticipate potential changes in patient status of management and communicates to attending physician.


• Provides follow-up calls for education and support, before, during and after the primary treatment to promote coping mechanisms for both the patient and family.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

• 1 year Care Manager/Navigator experience in Primary Care or Specialty Setting, required

• Current BLS certification, required


• Graduate from Accredited Nursing Program, required


• Current Nursing License in the State of Colorado, required


• 2 years Registered Nurse experience, preferred


• Experience working with, and sensitivity toward, special populations, including those with limited income, and limited education, and with non-English speaking individuals, preferred


• Full understanding and working knowledge of EPIC computer systems, preferred


• Experience with PC software applications in word processing, Word, and Excel, preferred


• Bilingual English/Spanish, preferred
