Hem-Malignancy Advanced Practice Provider

MINIMUM QUALIFICATIONS:

• Graduate of accredited Master's Degree Program in Physian Assistant Studies with PA-C Certification

                                                              OR

• Graduate of accredited Nurse Practitioner program with with Nurse Practitioner Certification for adults

• Licensed to practice medicine in the State of North Carolina
• Hematological Malignancy Experience Preferred

POSITION SUMMARY:  Provide direct patient care in the inpatient setting to patients during the pre-, peri- and post-chemotherapy periods in collaboration with Hematology/Oncology attending physicians and nursing staff.  Ensure continuity of patient care. The age-specific scope of practice is between 18 yrs and end of life.

RESPONSIBILITIES:

• Perform necessary diagnostic and therapeutic procedures (within the scope of practice of the supervising physician).  These may include, but are not limited to the following:

• bone marrow biopsy/aspirate
• bone marrow harvest
• lumbar puncture
• Hickman catheter removal
• skin biopsy

• Conduct physical exams, obtain patient histories, monitor patient progress
• Order appropriate medications, laboratory studies and diagnostic procedures.  Prescriptive practice will exclude chemotherapy.
• Document the patient's course of treatment at the direction of the supervising physician:

• History and Physical
• Problem List and Plan of Care
• Daily Progress Note
• Procedure Note
• Interval Notes
• Documentation of GVHD/Toxicities in Flow sheets.
• Discharge Summary

• Accept scheduled and unscheduled inpatients
• Facilitate daily rounds in both the inpatient and outpatient setting
• Collaborate with other program and non-program services, physicians and nursing staff in order to provide a longitudinal continuity of care between the inpatient and outpatient setting, and through follow-up
• Participate in meeting the educational requirements of students, medical  residents, fellows and other members of the health care team
• Collaborate with members of the health care team in helping to assure compliance with research related protocols
• Collaborate with the nurse manager in the identification of problems and their resolution in order to assure the providing of safe, but cost effective patient care
• Actively participate in program related committees and/or projects
• Provide a summary report on the status of inpatients to the BMT attending and medical resident on call prior to leaving each evening
• Assure proper charges have been posted and appropriate documentation provided
• Attend BMT Program required meetings

SITES OF PRACTICE:  The advanced practice providers associated with the Adult Bone Transplant Program will provide coverage and practice in the outpatient day hospital (North Pavilion) and home transplant patients seven days per week including weekends and holidays.  You may also work on 9200 ABMT inpatient unit.
