Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. Grade Determinations: Medical Instrument Technician (Vascular), GS-9In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments. Experience: At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates the knowledge, skills, and abilities to fully perform all vascular ultrasound tests and examinations and to function as a technical director of the vascular laboratory and provide oversight and/or guidance to other technicians. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to vascular ultrasonography. Knowledge of JCAHO and other regulatory requirements. Ability to communicate effectively and professionally with employees at varying grade levels. *Ability to manage and supervise employees. *Ability to provide, or provide for, staff development training. Preferred Experience: The vascular lab technologists will assist the vascular surgeons at the VA Medical Center in the diagnosis and treatment of a wide variety of disorders affecting the vascular system. The vascular lab technicians provide carotid studies, consistent diagnosis criteria and quality control for venous duplex scans, venous reflux determinations, assess for stenosis or occlusion of the mesenteric and renal arteries, assess for DVT of the upper extremities, preoperative vein mapping, intraoperative assessment of carotid endarterectomies and lower extremity vein bypasses, surveillance of vein bypass grafts, and segmented arterial pressure and waveform analysis with formal interpretation. Requires relaying information to the physician and patient care providers to provide diagnostic assistance when needed. Responsible for the evaluation of all equipment and the techniques for non-invasive vascular testing. Performs high quality ultrasound exams of the cardiovascular system utilizing B-mode, 2 dimensional ultrasound, Color Doppler and Pulsed Wave Doppler measurements. Independently make standard examinations and makes more in-depth examinations when abnormalities are present. Advises the vascular physician and ordering provider of preliminary results of the exam, recognizing significant findings that require emergent care and relaying information to insure proper treatment. Evaluates the nature of critical and emergent procedures and rearranges patient priorities to accommodate them. Maintains records of patients treated, exams performed, and ultrasounds taken in the department logbook. Process all worksheets and hard copy images, including the preliminary findings and presents to the vascular surgeon for interpretation. Orders, picks up, and stores supplies for the laboratory to ensure continued support for patient care. Reviews, evaluates and participates in the continuing education programs and refers to pertinent clinical literature with regards to improving the quality of examinations. Conducts preoperative vein mapping studies of upper and lower extremity superficial veins for procedural planning. Performs studies such as, transcutaneous oxygenation, ABIs/arterial waveforms/PVRs, carotid duplex, peripheral arterial duplex of native arteries and bypass grafts, AAA studies, AV Fistula/graft duplex, renal mesenteric duplex and venous studies.
