Details Posted: April 8, 2021 Location: Somerset, Kentucky Show Map Salary: Open Two vacancies located within the Lexington VA Health Care System at the Somerset Clinic in Somerset, Kentucky. This clinic provides primary care, along with limited specialty care services, and operates in a multidisciplinary PACT model with Monday through Friday business hours. The primary role of the physician is to provide comprehensive medical care to an assigned panel of veterans. Recruitment / Relocation incentive may be authorized for a highly qualified candidate. Education Debt Reduction Program (EDRP) is Authorized This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30, 2021. The initial cut-off date for referral of eligible applications will be October 23,2020, with subsequent cut-off dates every two weeks. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. Candidates must have an internal medicine or family practice background. Please join our team if you have a passion for serving veterans and a desire to work in a newly built, state of the art outpatient clinic, settled amongst beautiful scenery. Duties include, but are not limited to: Provide total care of the Veterans' medical needs by performing history and physical exam, ordering appropriate lab tests, radiology studies, screening studies. This includes evaluating those tests, developing a treatment and follow-up plan to include medication, medical equipment, and home-based interventions within the guidelines laid down by the supervising physician. This care will also include the outpatient follow up visits, post discharge follow-up and managing care with specialty and sub-specialty medical providers. The patient care will be provided through face to face visits and/or other modalities (Telehealth Services, telephone clinics, secure messaging, etc). Assist in the preparation of the patient and/or his/her family to include, but not be limited to, teaching (to include health promotion and disease prevention activities), accumulating data and medical problems, and presenting patient centered discussion and whole health of medical problems for appraisal and scheduling patients with the appropriate Primary care department services. Responsible for completing patient encounters, notes, orders, consults, and correctly coding visits in accordance with coding guidelines. Act as the focal point of the designated outpatient clinic by being the contact person for all other consultant services regarding patient care and providing consults to other services. Provide assistance to the Primary Care Department with regard to duties to assist the service that may be requested. These may include, but not be limited to, assisting in data assimilation for VA reports, quality assurance programs, or others as determined by the Chief of Primary Care or Chief of Staff. Maintain skills, proficiencies, and professional knowledge required for licensing and clinical scope of practice. Provides support to APRN's working in clinic. The Physician will be required to attend staff meetings, may serve on committees, teams, workgroups, and complete peer reviews as requested. The Provider will complete CME, VA Training ie. HIPAA, Security training, and other mandatory trainings as assigned in TMS.
