Details Posted: April 3, 2021 Location: Augusta, Maine Show Map Salary: Open The Clinical Pharmacy Specialist performs duties of a clinical and teaching nature, including direct patient care, routinely functioning with minimal supervision as an independent practitioner operating under a scope of practice. The Clinical Pharmacy Specialist participates in said activities upon a consult referral from VA Maine Healthcare providers, thereby enhancing the delivery of clinical pharmacy services to PACT and Primary care teams. Duties include but are not limited to: Using advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics, s/he serves as an authority on pharmaceutical care in specialized and routine areas of clinical pharmacy practice for medical staff, nursing staff, and pharmacy staff. Responsible for the evaluation, monitoring and medication management of Veterans referred by consult to the Clinical Pharmacy Specialist for chronic disease state management. This includes the initiation and prescribing of appropriate pharmacotherapy and laboratory monitoring related to national guideline-based treatment of diabetes, dyslipidemia, and hypertension and other chronic disease state medication management as requested by VA Primary Care Providers. The design of a therapeutic regimen or plan, based on patient specific information, which includes establishing a therapeutic endpoint, selecting an appropriate drug therapy, establishing an appropriate dosage regimen and providing appropriate and timely lab monitoring of related to all prescribing activity Responsible for the evaluation, monitoring and management of Veterans referred by consult to VA Maine Healthcare Anticoagulation Clinic (ACC). This includes warfarin dose adjustment, determination of frequency of laboratory follow-up and management of "bridging" with low molecular weight heparins at the initial direction of VA Maine Primary Care and Specialty providers. Other ACC functions include full participation in quality management initiatives and monitoring of anticoagulation therapy, patient education and serves as an anticoagulation therapy information resource to all VA Maine Healthcare providers for Veterans enrolled in ACC. Upon request of Primary Care providers, deliver medication management services to Veterans including; 1) Review of medication regimens of all referred patients and noting for the VA provider any findings that might require action that include but are not limited to: a. Inappropriate dose, dose interval, or dosage form b. Drug and/or pharmacologic duplication c. Adverse drug-drug, drug-lab, or drug-food interaction d. Adverse drug reactions (reports adverse drug events (ADE), near misses and, medication errors in alignment with VA ADERS reporting program e. Inappropriate therapeutic regimen f. Misuse of medication, i.e., abuse, overuse or underutilization 2) In cooperation with the Primary Care Provider, recommends alternative drug therapies designed to reduce cost of medications while continuing to ensure optimal therapeutic response 3) Serves as drug information resource for Ambulatory Care/PACT providers answering inquiries regarding therapeutics, drug interactions, dosing, side effects, formulary options, etc. 4) When assessing or providing patient care the clinical pharmacist will consider the age of the patient as it relates to ability to understand directions, provide objective self assessment or any other medical milieu appropriate to the Veteran's overall treatment plan. 5) Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the Veteran is not taking 6) Participates in the medical center medication utilization evaluation (MUE) program 7) Manages various actions, as appropriate, for medication orders to include flagging prescriptions with appropriate VA provider follow-up, managing view alerts and, assigning electronic medical record surrogates during times of absence from the medical center. Participates and/or supports Pharmacy Service research efforts Participates and/or initiates quality management efforts within Pharmacy Service, including analysis and management of a clinical practice within an area of expertise or as otherwise assigned. Assists in conducting and reporting internal audits as required including VA ADERS reporting. Maintains all pertinent professional and administrative records as necessary. Completes all encounters, charting and documentation including scheduling, reviewing consults and finishing encounters, to meet timeliness standards Participates and/or supports Pharmacy Service-initiated cost/benefit programs in support of VA Maine Healthcare's efforts at maintaining stewardship of resources The incumbent will be expected to provide follow-up on reported adverse drug events in the ambulatory care and inpatient medical/surgical patient care areas or other areas within the medical center when necessary. The incumbent will also be expected to submit completed adverse drug event report forms to the Clinical Coordinator of Pharmacy Service as part of the follow-up process.
